How to get started with a referral:

Book a double appointment
Visit your GP and request a Mental Health Care Plan (MHCP). It is often helpful to book a longer “double” appointment to allow enough time for this assessment.

Request a referral letter
In addition to the MHCP, your doctor must provide a referral letter addressed to Monica Molyneaux

Send your documents
Once you have both your MHCP and your referral letter, please email them to our admin team at: info@destinationhope.com.au.

Confirmation
Once we receive and process your documents, our admin team will contact you to finalise your intake and confirm your first appointment.
Note
Your referral letter must be dated on or before the day of your first session to be eligible for Medicare rebates.
